LOWER-COMPLEXITY FERTILITY CARE
Timed intercourse: ovulation tracking and planning
Timed intercourse uses cycle information, ultrasound and—when appropriate—hormone testing or medication to identify the fertile window. It may be reasonable for selected patients after the main female and male factors have been reviewed.

Individual cycle review
Timing is based on the actual cycle, not only on an app estimate.
Proportional monitoring
Ultrasound, tests and medication are used only when clinically useful.
Clear reassessment point
The plan states when to continue, change strategy or investigate further.
WHO MAY CONSIDER IT
When timed intercourse can be discussed
It is generally considered only when ovulation, tubal status, semen findings, age and time trying to conceive support a lower-complexity approach.
Ovulation timing
Useful when cycles are irregular or the fertile window is difficult to identify.
Selected unexplained infertility
May be considered for a limited period when the basic evaluation is reassuring.
After ovulation treatment
Timing may be coordinated with medication and monitoring while controlling the risk of multiple pregnancy.
THE CYCLE
What monitoring may include
Baseline review
Cycle history, pregnancy history, ultrasound findings and prior laboratory results.
Follicular monitoring
Ultrasound or hormone testing when needed to estimate follicle development and ovulation.
Timing guidance
A specific plan for intercourse and medication, including what to do if the cycle changes.
Outcome and next step
Pregnancy testing at the appropriate time and a defined point for diagnostic or treatment review.
DECISION SUPPORT
What timed intercourse cannot correct
It does not open blocked tubes, overcome severe sperm limitations or reverse age-related egg changes. Repeating cycles without a reassessment can delay a more suitable option.
- Confirm whether at least one fallopian tube is open when clinically indicated.
- Review semen findings rather than assuming infertility is only a female factor.
- Discuss cancellation criteria if medication produces too many follicles.
Important limitation: Accurate timing does not guarantee fertilization, implantation, pregnancy or live birth. Your clinician should explain the expected benefit and the number of attempts that remain reasonable.
Ask whether cycle monitoring fits your diagnosis
Send your cycle history and available test results for an initial orientation before planning travel or treatment.
